Meet Tara l. Cook registered practical Nurse.
- From an early age, I knew I wanted to help people. Supporting others in achieving their best possible outcomes has always been at the heart of my work. My nursing career began in 2007 in a hospital setting, where I worked in both palliative care and rehabilitation, helping patients regain independence and reach their personal goals. For the past 15 years, I have served as a community nurse, delivering care across a wide range of areas including wound care, medication management, cancer support, post-operative care, and chronic disease management, with a particular focus on diabetes.
- I’ve been privileged to work in diverse areas of nursing, becoming well-versed in many aspects of care. One of the proudest moments of my career was receiving the Home Care Hero Award from the Province of Ontario—a recognition that means so much to me because it came from the voices of the clients, patients, and care team members I’ve had the honor to learn from and walk alongside in their journeys.
